If Customer wishes to restrict Sho-Me Technologies, LLC, from using Customer’s “Customer Proprietary Network Information” (CPNI) to offer Customer types of service other than those to which Customer already subscribes, Customer may submit a completed CPNI Opt-Out Request Form.
